When 5+ users edit concurrently, revision gaps trigger the Stale state
which throws 'Ignored server-side change.' on incoming operations. This
causes document divergence, silent data corruption, and eventually
force-resets that discard local edits.
Changes:
- Stale/StaleWithBuffer.applyServer: buffer incoming operations instead
of throwing, apply them sequentially after missing ops arrive
- Stale.applyClient: carry pendingOps to StaleWithBuffer on transition
- editor-socketio-server: retry-based force-reset (3 failures) instead
of immediate nuclear reset on any OT error
- hex2rgb: defensive null checks for old non-hex color records

The hex2rgb function seems to previously have been available globally.
It probably got lost in the great Webpack refactoring and nobody noticed
that.
This copies the function into its own file (to make importing it easy)
and adds an import in index.js.

Right now, the ToC has an undefined variable i that was an index in the
original ToC code. Since the major rewrite in
4fe0620853 it's a recursive function
without this index. The variable `i` was wrongly copied into its current
place from the old code.
This patch replaces the variable `i` with the index of the header
element. Fix the undefined variable problem.

The ToC generated broken HTML with unclosed `<li>` tags. This got fixed
as well as some minor optimisation and adding list elements for the
subentries so the elements appear in the ToC while scrolling.

This replaces the existing iterative implementation of the ToC
generation with an recursive one.
This also solves the problem of skipped headers which causes wrong
leveling of them.

Right now the full title of an element is may not shown as the space of
the ToC is limited. With this path it'll be shower on hover and this way
provide more useful information.
